Hi Jason,
is the Pane where you hit the hotkey a pane which allows to edit (and paste) text?
Has this Pane also the Focus?
Did you select the SQL to be formatted with the mouse?
The Hotkey solution is internally sending the keys "ctrl+c" (copy to clipboard) and after the formatting activity it sends "ctrl+v" (paste from clipboard).

thanks for your feedback. Indeed you found a bug. The root cause is that there is a tab char following the /* on the next line. Please download the fixed version from the download page. Please let me know if it worked for you.
This new version also contains your feature request of highlighting the Profile buttons.

Hi Bill,
when no profiles are loaded for the hotkey feature then SQLinForm formats with its default values. One of the default value is to put any SQL < 80 chars on one line.
How to create a formatting profile?
